OBJECTIVE/OVERVIEW: The Community Health Worker will provide patient education, outreach, scheduling, registration, and care coordination around healthy practices and lifestyle across the lifespan. The position may require travel and events’ participation in Minneapolis and surrounding areas. This position works within the care team to ensure that patients receive effective, understandable, and respectful care and resources compatible with their cultural health beliefs and practices and preferred language. This role also supports Mobile care activities, including mobile events and supporting remote and virtual care for patients and community members.
ESSENTIAL FUNCTIONS Patient education & Care Coordination (30%): â— Provide education to patients on preventive care, basic orientation on chronic diseases management, culturally relevant lifestyle adjustments, co-lead and facilitate focus groups both at CUHCC as well as in collaboration with community partners. â— Assist with patient follow up, referrals and resource needs. â— Assist with managing patient panels Outreach (30%): â— Conduct community outreach on specific topics. â— Connect community members with CUHCC services and programs, as appropriate. â— Provide health information, and/or referrals, both at CUHCC as well as at community events. Patient Registration/scheduling (20%) • Registers and schedules appointments that support outreach activities. • Supports scheduling duties at mobile care events. Data collection/analysis (10%): â— Collect and input relevant clinical and outreach information into the central database/electronic health record. â— Identify or act on follow up needs on specific health areas. Communications (10%): â— Assist with the development/preparation of educational materials. â— Present health information to audiences. â— Ensure feedback on community needs communicated to appropriate channels.

REQUIRED QUALIFICATIONS: â— BA/BS or a combination of related education and work experience to equal four years â— Excellent oral communications skills in a primary CUHCC language â— Excellent oral and written communications skills in English â— Demonstrated proficiency in using computers and related software, including past experience using an electronic health records system â— Willingness to work with diverse populations from various economic backgrounds and age groups â— Demonstrated ability to work in a fast-paced environment with a variety of customer service needs. â— Must have reliable transportation â— Must have a valid driver’s license
PREFERRED QUALIFICATIONS: â–ª Community Health Worker certificate (or near completion) â–ª Knowledge of and experience educating on preventative care and healthy lifestyles. â–ª Demonstrated ability to work collaboratively with team and partners to plan and implement program activities. â–ª Self-starter with ability to juggle multiple priorities and meet deadlines â–ª Previous experience working with community health â–ª Strong skills and experience in patient confidentiality and Electronic Health Records â–ª Minimum 1 year of experience working or volunteering in community settings â–ª Minimum 2 years’ experience working in clinical setting.
